Does whatever glues your installation of ClamAV to your MTA do this for you? I would expect that MIMEDefang would. In fact IIRC MD decodes the message, put it into a temporary directory, and directs ClamAV to check the decoded files there. You might look into that and see for sure. That's what I recall though.
