You haven't said what version of clamav, clamav-milter and operating system you are using. Assuming it's clamav-20030403, clamav-milter 0.53 and Linux/x86, the message "not scanning outgoing messages" appears when the source IP address is 127.0.0.1 (i.e. the local machine) and the flag "-o" hasn't been added to /etc/sysconfig/clamav-milter.
