clamd scans individual files you tell it to scan (see the documentation to learn how to tell it to scan something), without having to initialize the entire virus database for each scan (like clamscan). Rather than running clamscan for each email attachment we need to scan, we just tell clamd to scan the indiviual files and report back. You can process a much higher volume of files this way.
