Hola Rodrigo,
Quita el "-H cvbuilder" de la linea check_command y déjala así:
"check_command check_mysql -P 3306 -u root_passwd"
En "check_command" no es necesario indicar el host ya que ya que en la
definición del servicio hay una linea para ello:
"host_name cvbuilder"
