What version of SDL is used on respectively OpenBSD, FreeBSD and
Archlinux ?
What window managers have you tried besides mwm, dtwm and fvwm ?
You say that both abuse and c-dogs work with CDE. Have you tried
lbreakout ?
It is an SDL game, and I remember that it was working with the LGPL CDE
before 2017.
I know that CDE is not EWMH-compliant, so your problem could be related
with SDL expecting
an EWMH compliant window manager maybe because the game wants to run
fullscreen.

suggests that setting the environment variable
SDL_VIDEO_X11_LEGACY_FULLSCREEN to 1
forces the legacy fullscreen support. Does that improve the behavior of
the game under dtwm/mwm ?

The same behaviour does not happen in say some of the more conventional
WMs and DEs, I believe it to be a new bug with sdl, as I remember
playing sdl games on CDE in the 2000s on solaris and it was fine.
A game that you can use for an example that is both free, and easy to
obtain would be Xonotic-SDL. I have tested it on CDE for openbsd,
archlinux and freebsd with the same results. Its window does not open
properly even in windowed mode. Yet it will launch perfectly in a more
modern WM, so I think there is an issue with SDL games and dtwm, fvwm,
mwm and other WMs of this nature such as twm, vtwm, ctwm, etc.
Again, a few games I can think of that have the issue (pretty much any
sdl game)
xonotic-sdl (easiest to get) doesnt work
DevilutionX doesnt work,
abuse works
c-dogs works
i do not know how it got like this when i could run quake sdl on the
original cde, but it would be nice ot see a fix as part of the effort
to return cde to its original glory, sdl devs certainly arent going to
care about some obscure WMs and games
