That's not how this works... :)
If you are going to support this, then you need to essentially re-add it
as a new component... And it needs to be complete and functional at
that time, and apply to whatever branch (master or cde-next) you are
targeting...
You can go back to an earlier version of CDE to obtain things you might
want to re-add back (in another checked out tree), like the .udb
entries, but you can't send a patch and then later mention that you want
some other patch reverted to make your patch work. That is madness.

The patch requires the disabling and deleting commits to be reversed.

I'm not sure what I'm supposed to do with this. These have been removed
from master (via cde-next) some weeks ago, and would not build anymore
even with this patch... In addition, you mention yourself that it's not
done, so... Why did you submit it? :)
Perhaps you need to occasionally rebase your topic branches with master
or cde-next depending on what branch you are targeting? Have you been
doing this?
Certainly, this should be done before submitting a patch - that may
explain why some of the patches you submitted would not apply.
I'm not going to merge this.

My fixes to this program arent done, but I wanted to put this out before
the code moved too far ahead for my changes (as did btree_berkeley),
some of the changes include:
removing register keyword
updating to ansi functions,
using function prototypes,
minor optimizations,
removing useless defines
replacing custom headers with system headers (I still need to replace
pcf.h, but I can't seem to find where the system header is)
fix some compiler warnings (including all surpressed ones for this program)
minimize use of goto
