Great work Cody! I performed a little research and determined that the get_categories() function I was using is not supported on versions before 2.1 and wasn't really useful until 2.3 I have included a possible fix by simply replacing the function with get_terms() and I need you to try again with all versions to see if it works and does not break any of the newer versions of WordPress.
