Did you figure out? I am having a similar problem. It seems like the default layout is missing in the action and it renders only the view file for that action. I also have a couple of forms on the page with links 'onclick=document.form_name.submit()'
As reported originally, it happens randomly. The page works fine most of the time, but once it gets messed up, it stays like that for 5-10 minutes. I see this on both IE 7.0 and FF. I wonder if it has something to do with cache rebuilding or something else periodic.
