Thanks for reporting this bug precisely as well as the other
two. Be aware that lilypond-book's Docbook support is not
currently (as far as I know) exercised in the main build, so
there may be other quirks, and I suspect it will be hard to
find a developer in the current pool to work on it. I suggest
that you open issues on the tracker directly at
https://gitlab.com/lilypond/lilypond/-/issues,
preferably providing patches to solve them ;-)
