Notation Appendixes, as I understand it, are 'auto generated' so to speak from within the 'code' (i.e. the numerous files used to build LilyPond) as opposed to the 'rest' of the Notation Manual which is mainly entered manually - if you see what I mean. Yes it would be lovely to have all the commands documented in the Notation Appendix but that is not trivial (at least not for a lowly documentation editor as myself) to do. I also believe that there is already an existing Tracker to 'document all our commands' via the 'autogeneration part' of our doc building. The first index link just needs an additional suitable example to add after \markup perhaps? The second and third links is to different lines of your 'case B #3' (see my comments above).
