Thanks for the report, Louis. I also searched the issues list and found a more recent one from February. It reports a collision between a breath mark and an accidential: http://code.google.com/p/lilypond/issues/detail?id=2354 So, I think this is a known issue. I'll link to your post from the tracker so that your case can be included when the devs verify their fix.
