I am sure this is totally not an ISO-certifiable process, and it has the rather unfortunate property of discouraging exhaustive documentation of the current state by whoever wants to tackle it. But since this process takes time, and more time the worse the current state of affairs is documented, improving the documentation is still a worthwhile job if someone likes working on it.
