That's the manual for GRUB Legacy not GRUB 2.
But it should still work if device.map lists fd0.
Else just use grub-mkfloppy (and in newer versions grub-mkrescue) to generate
a floppy image which can be directly copied with e.g. dd to /dev/fd0
