A discussion on the `linux-man` list prompted an idea. The formatter doesn't
know which table entry made a table region reach its maximum width, but the
preprocessor _does_, and could stash the column/row information in registers
or a string as a GNU _tbl_ feature. The diagnostic message _tbl_ embeds could
then retrieve this information.

Implementation: use a typical "high-water mark" approach; each time we have to
increase `TW` beyond its current value, update the culprit row/column
information.
