Follow-up Comment #6:

It's not quite what Branden envisioned in the original submission, but Deri's
Zseq.pl seems really useful, and does the job described in the Summary.  So
I'm marking this ticket as "[PATCH]": we now have a tool that could be shipped
and subsequently improved, for instance to utilize structural information the
macro package or user supplies in the source document.

Deri's sample command uses -Tpdf, but Zseq.pl seems to work just as well with
-Tps, which gives it one advantage over the PDF-only page-pointer manipulation
described in comment #4.
