Details:
I am unsure of where this error originates, pdfmark.tmac, grops, ghostscript,
or ??? Please check the attached image to see where the hotspot actually
appears - it is not on the item. The vertical distance between the hotspot
and where it should be increases with .pdfhref entry
Please see attachments for test files,
