Remove remaining init.texi references

Hi.

There are still a couple of references to (then generated) init.texi present
after bfd_init move [1].
  - At bfd/doc/bfd.texi
        This causes build to fail with "bfd.texi:246: @include: could not find
init.texi"
  - At .gitignore

Attached a proposed patch that fixes these issues.
