Yes. I tried to follow the link in order to answer the survery. The first thing that happened was that I was asked to log into google. For me it is not clear why I would need to log into my own google account (and that way to provide my credentials) only in order to fill out the survey of a person that I do not know.
These days I receive many phishing emails that ask me to log in here, log in there, and so on (typically with unusually long URIs) so I felt obliged to warn the other list members. I think you should change the setup of your survey in a way that allows users to fill it out without providing their credentials (or even anonymously).
