Not being a Windows person, I don't know all that much about how NVDA works. It 
may be, however, that NVDA is expecting brltty to be running as a Windows 
service rather than as the result of manual invoaction.

There are two .bat scripts that come with brltty on Windows: install.bat, and 
uninstall.bat. Please try running install.bat to get brltty running as a 
service.
