Hypothetical situation: an USA aircraft in Colombia, waging war against some insurgents or cocaleros, violates the air space of Venezuela. Venezuela hails it, but it doesn't respond, either because they don't fcare or because they don't hear. If Venezuela tries to bring it down, it's _not_ an act of war.
I think any decent definition of "attack" would not show that Iraq attacked the USA neither before nor after GW1. As far as I remember, the only attacks against the USA since 1990 or so were terrorist attacks in USA soil or in USA embassies. I don't count attacks against war units that are posed to _attack_ other countries as attacks against the USA, but as acts of defense from USA attacks.
