Whatever you do, don't look down to distract yourself. Stuff ends up on
the floor. Stuff you probably don't want to see if you're about to pass
out. (A friend of mine found that out the hard way.)
Also, if there's going to be an episiotomy, go to someplace where you
can't see it. Next to your wife's shoulder, getting your hand crushed in
hers, might not be a bad spot. (Dan's advice.) Once that's done and the
scissors have been put down, it won't be as bad.
