Absolutely. Nothing worse than "Dear Sir/Madam" or "To whom it may concern" on an application.

3.  This question is one that I am personally on the fence with - My
supervisor at the DoD is really great about supporting people when they
have opportunities to move up and advance in position, job or salary.

If you do this, it would have to be in conjunction with pointing out that you are going from contracting to a permanent position. In that case, I think it would show that you know you can do the job and that others will support that opinion.
