It is not a bug, it means you missed at least one monster or item on the ground. It is also not crucial to the game: you can complete it just fine missing a couple, as long as you have multiple of each color at the end. I would suggest going back through each room and using the d command to make sure you didn't miss anything.

Sometimes, I am not able to find all the data wafers on the first level. I get to the elevator, and when I check my status, sometimes it will say Data wafers not found, 1, and I try to retrace my steps to find out where the heck the last data wafer has got to. It seems to be random in each game. This time, At the end of level 1, I was able to grab the timed blast mine from the boss, and blast a whole in the wall to get the message and kill the 2 dogs, I know that each monster will often drop an item, and I've looked high and low for that last data wafer, but I can't find it anywhere.
