Well, are there any logs - IMAP Server side or apache side - which may help troubleshooting?

Also, is this a true IMAP server or a Windows IMAP Domain server? Windows servers use NTLM and the handshake is somewhat different depending upon which Exchange Server version you are using.

Lastly, there is an Apache users group which may comprise more experienced Apache<->Foriegn Authentication professionals.

My NTLM Perl experiences only cover WinXP Domain authenication and I had to custom write C code to get it to work.

(No, I cannot share as it was a work for hire and my old employer owns the code ... However around that same time Mark Bush wrote Authen::NTLM. That may give you some additional help.)
