DBI::Shell is part of the DBI distribution. There is a bug tracker at http://rt.cpan.org/NoAuth/Bugs.html?Dist=DBI, mailing list information at http://www.perl.com/pub/a/language/info/mailing-lists.html#db, and, as with any module on CPAN, you can email the author using his CPAN ID (<id>@cpan.org). I wouldn't suggest the latter if you want results, however; try the mailing lists. You should also make sure you have the latest version of DBI installed, to verify the bug is not already fixed.
