The last part of your script looks like this

            </form>
        </body>
    </html>
    END_HTML
    }

It should look like this

            </form>
        </body>
    </html>
END_HTML
    }

In other words, the END_HTML token needs to be on its own line with a return
immediately before and after it and no extraneous characters, including
spaces. Does that make sense?
