I've noticed this often happens when a volume is written to media that
doesn't belong in its pool.
For example, here in my environment, we have a media for each day of the
week, where differential backups are recorded, and we have other media,
used on weekends for full backups.
If a full backup is written to media intended for differentials, by a
fileset change for example, that full backup will never be purged.
