Thq query output looks suspicious to me -- do you expect the file to be stored 5 times in the same job?
It might be interesting to see the output from "list jobmedia jobid=1100162" and query 7 (List Volumes used by selected JobId) for 1100162. If you are using mysql, then I suggest running mysqlcheck abd myisamchk if appropriate.
