Yes, but not live, you would have to shut down the vm, backup the
file, startup the vm again.

I have heard that it is possible to take a VSS snap of a VM HD in a
way that links to the VSS writer in the guest to create a self
consistent "snapshot" of a VHD, but I have not seen it done.
