You find it in the example subtree of the source code - I would recommend to use 5.0 source to built it, since it's included in the ./configure routine, which makes creating it much easier... just ./configure with --enable-client-only, change to src/libs, 'make', change to the Nagios plugin directory examples/nagios/check_bacula and 'make' again. Minimal documentation when called without parameters. check_bacula *should* work with all Bacula versions, though I admit I heven't trsted on anything except 3.0 and development versions shortly before 5.0.
