Sometimes (less than 1 time per 3 months) I see this with my archive. And the solution is to stop bacula-sd. If that did not work unload mt modprobe -r mt If that did not work unload the scsi driver then reload it. for me this is sym53c8xx so modprobe -r sym53c8xx modprobe sym53c8xx usually does the trick.
