No, I think this is OK. The SD has failed to *read* from the tape.

If this is a new tape, you can get an I/O error when you label it for
the first time. Bacula tries to read the tape to see if a label already
exists on it (this is a safety check to guard against accidentally
overwriting a tape). If the tape is blank, you get an error from some
devices when you try to read it.

The label appears to have been written OK. If you are in any doubt, delete
the tape and test it with tar or cpio. Then you can label it again (as long
as you have overwritten the Bacula label on the tape).
