Ok, this might be.
And this might explain the two wrong files.

However, I'm getting the append problems on every tape, so it is 
possible that I'm seeing two different problems.

This is strange.
I do not use any script to initialize the tape, and, apart from the 
tests you suggested and I did, bacula is the only application that's 
using this device.
Are this settings permanent over reboots and power offs?

I'm controlling the machine via ssh, so I can't read the console.
In the logs I found:

Feb 16 13:04:50 xxxxx kernel: (sa0:sym0:0:5:0): tape is now frozen- use 
an OFFLINE, REWIND or MTEOM command to clear this state.

I can't tell exactly when this was logged though; is is possible it was 
during the eod command.
