I've seen input/output errors on my tape drive in 3 instances- 1) yes a failed tape 2) there is no tape in the drive 3) the joy that is errors on your SCSI chain (assuming your tape drive is SCSI attached. try sacrificing a goat to your SCSI host bus adapter...)
