Well, start by including the Job btape test output just before the failure.  
Actually seeing it may give me some ideas.    

When upgrading, I think Bacula overwrites the mtx-changer script, so if you 
had previously tweaked it to work with your autochanger, there is a good 
chance those tweaks are lost.  I recommend taking a careful look at it.

Finally, you can try setting a debug level of something like 100 on the SD or 
btape, that may give some more info on what is failing.
