Do you have any messages related to the SCSI subsystem in the system log? I suspect this is either a hardware problem (which you can verify using, for example, dd to write lots of stuff to the tape in question) or a driver / module problem. You'd have to tell us which SCSI HBA you use, which module is loaded, diagnostic output from 'cat /proc/scsi/scsi' ant the like. Perhaps someone observes something unusual.
