Do you have any messages related to the SCSI subsystem in the system log?

I suspect this is either a hardware problem (which you can verify using, 
for example, dd to write lots of stuff to the tape in question) or a 
driver / module problem. You'd have to tell us which SCSI HBA you use, 
which module is loaded, diagnostic output from 'cat /proc/scsi/scsi' ant 
the like. Perhaps someone observes something unusual.

Also, did you use the tape drive and tapes with different software 
without any problems?

Might also be a problem in the software - did you compile yourself, for 
64 bit or 32 bit architecture, with all the correct libraries etc?
