It also shows the tape reaching EOM at only 3.4Gb.

Assuming the raw capacity of the tape is higher than this then there is a 
problem with either the tape or the drive.

I was seeing this on LTO2 media recently. The drive wasn't showing a 
"cleaning required" light and running a cleaning tape didn't make any 
difference. In the end we swapped it out under maintenance contract and 
the problem went away.
