is this since you upgraded from a prior version to 1.38.5?
I had the same problem (search for "autochanger does not (auto-)load").
The solution for me was just to delete the volume that was needed and then 
recreate it. I think there is a problem with the database. After I recreated 
only the first tape needed by bacula (the tape which bacula should load first), 
it worked. Using the mtx-changer-script does not work very well for me, so I 
decided to use pjc (ftp://ftp.lysator.liu.se/pub/unix/pjc) which now works 
perfectly.
