Same here with version 1.36.2 on Debian. Kern recently told that there is a 
nasty bug in Linux kernels 2.6 that the storage daemon sometimes has 
problems with streamers. (A tape needs to be inserted when the storage 
daemon is started because otherwise a whole backup job fails when a new 
tape is requested.) This doesn't happen here with disk/file backups 
though. But although I have to manually unmount and mount the new tape at 
least the old tape gets ejected correctly. It annoys me a lot, too.

I probably can't help much more but can you tell your operating system 
environment and Bacula version anyway?
