what you describe looks like a driver or hardware problem to me. Alhough I can't be sure, I'm not able to imagine anything bacula-related leading to such problems.

If you can reproduce the problem now, chances should be good that, after a shutdown, complete power off, and restart of server and tape drive he problems should vanish. After all, your setup worked long enough without problems already.
