Alan,

I seem to recall having an issue like this but it didn't take more than a 
few tape swaps to find it.  The solution in my case was to set offline=1 in 
the mtx-changer.conf so that bacula always issues an "mt offline" command 
before unloading a tape.
In addition, in my bacula-sd.conf I use
AlwaysOpen=no
CloseOnPoll=yes
VolumePollInterval=300
