The firmware really did not load. But the reason was not a wrong
version of madfuload, but a missing update in the /etc/fstab when
upgrading from SuSE 10.0 to 10.1. The entry

usbfs         /proc/bus/usb usbfs noauto 0 0

had to be replaced by

/dev/bus/usb  /proc/bus/usb usbfs defaults 0 0

I finally found this information on the sourceforge homepage of "USB
MIDI Firmware Loaders" in a bug report.

For some reasons I missed the error messages generated by madfuload:

madfuload: --device not given, and DEVICE not set

(I did a grep for usb).
