I have a Griffin iMic which works perfectly. I think it does everything you want, it's also cheap: mine is the older silver model and cost $35. Haven't tried (or seen) the newer white one but I'd be very surprised if it didn't just work. The quality is pretty good although, for some odd reason, I can hear hum from my laptop power supply when I connect it to an amplifier instead of headphone. I suspect that's more a problem with the laptop rather than the iMic.

I do not believe it is coming from the iMic or the laptop. That is one of
thehuge advantages of usb-- the soundcard is separated from the inside of
the computer where there are loads and loads of noise sources. And the
connection is digital-- analog noise sources do not bother a digital steam,
unless the noise is up at the freq of the digital stream.

I would look at ground loops in the connection between the soundcard and
the amplifier or at the amp itself. I would not look at the laptop.
