Thanks very much. You and Volker came up with the same advice and it was 
correct. When people have said "check the mixer settings" I have tended to 
look at the sliders harder than the buttons. A mistake in this case.

Another piece of advice I have just found very useful for solving mixer 
problems has been to use gamix to tweak the settings. It is visually very 
ugly, but it gives more detail than other mixers. This can be very useful. 
For instance it clearly distinguishes between mic input level and mic 
straight-through output level, whereas kmix does not.
