Generally "at the time of creation".  That used to be in the rules, now it's 
game custom (or maybe there's a precedent).

You need to specify "without 3 objections" in the intent.

Announce your intent to do so Without 3 objections, then 4-14 days later
(assuming you don't get 3 objections), announce that you do so.  You can
assign the judgement at the same time you announce you do so if you 
like.
