IIRC, the initial distribution was ineffective because it missed specifying a co-author; it was effectively distributed a couple hours later with the same proposal number, and votes referring to the proposal by number should unambiguously identify the decision that was initiated effectively (even if sent as replies to the ineffective distribution).
