The first clause is "when the CotC recuses a judge with cause"; I'd
assume that's when e can do so, at the time of recusing the judge.
That was my intent, yes. Something like one of Zefram's constructions
could be applied to self-recusal with cause; I left that open because
there are multiple ways to implement it, none of which seemed obviously
better than the others.
