Ive had to do this before It can be very simple as X company agrees to provide 1Gbs internet transit to Y company and have both parties sign it. I have also just got the person that was an Arin contact for the upstream to confirm the agreement by SWIPing a block of addresses to my org.

This isn’t something you write - this is a copy of an invoice or contract with one of your upstream providers showing you are in the US.
