Upon checking the attached logs, it seems that the Add Lead Form Extension feature is available to allowlisted accounts only. Also, note that Lead forms require that the Terms of Service have been agreed to before mutates can be executed. That being said, as for your allowlisting concerns, I'm afraid that this is already out of our team's expertise. We suggest that you reach out to your assigned Google representative instead.
