The USER_PERMISSION_DENIED <https://developers.google.com/adwords/api/docs/common-errors#AuthorizationError.USER_PERMISSION_DENIED> error occurs when the link between your manager and client accounts have been removed or that you OAuth2 credentials doesn't match the customer ID specified in your auth.ini file. I would recommend that you try the GetAccountHeirarchy PHP example <https://developers.google.com/adwords/api/docs/samples/php/account-management#get-the-account-hierarchy-under-the-current-account> to see which accounts are linked to your MCC account.
